ABSTRACT

Objective: To reduce environmental biohazards, technicians should be educated with safe biological waste disposal procedures and dental material recycling. The present study assesses the retentive force, deformation test and XRD analysis of re-injection acetal resin clasps. Material and Methods: specimens were prepared for clasp retention test injection according to the manufacture's instruction for acetal resin. The first group represent control group with (100% New) and other five groups as experimental groups (group II (25% old +75% new), group III (50% old + 50%) new, and group IV (75% old + 25% new) with different percentages between new acetal materials and old acetal materials, while two last groups with several times (group V (100% old) and group VI (re-injection two times)) only used old acetal materials. The X-ray diffraction (XRD) device used to give structural information for the materials to be tested. The study data were analyzed via One-way ANOVA (LSD) at a significant P-value of (p≤0.05) and a confidence level of 95%. Results: After comparing the results, a significant improvement in the retentive force of acetal clasp specimens after re-injection two times there was found high mean value in group re-injection two times and lower mean value recorded in control group. In XRD patterns of there was no diffraction peak refers to crystal structure of acetal resin. Conclusion: the present study findings concluded that the recycling of acetal resin enhanced the retentive force of acetal clasp and not affected on the crystal structures of material(AU)


Objetivo: Para reduzir os riscos biológicos ambientais, os técnicos devem ser educados com procedimentos seguros de eliminação de resíduos biológicos e reciclagem de material dentário. O presente estudo avalia a força de retenção, teste de deformação e análise de DRX de grampos de resina acetal reinjetada. Material e Métodos: as amostras foram preparadas para o teste de retenção dos grampos e a injeção realizada de acordo com as instruções do fabricante para resina de acetal. O primeiro grupo representa o grupo controle com (100% novo) e os outros cinco grupos como grupos experimentais (grupo II (25% antigo + 75% novo), grupo III (50% antigo + 50% novo) e grupo IV (75% antigo + 25% novo) com porcentagens diferentes entre materiais de acetal novos e antigos, enquanto os dois últimos grupos (grupo V (100% antigo) e grupo VI (reinjeção duas vezes)) utilizaram apenas materiais de acetal antigos. O dispositivo de difração de raios X (DRX) foi utilizado para fornecer informações estruturais dos materiais a testados. Os dados do estudo foram analisados por ANOVA um fator (LSD) com um valor P significativo de (p≤0,05) e um nível de confiança de 95%. Resultados: Após a comparação dos resultados, foi encontrada uma melhora significativa na força de retenção das amostras de grampo de acetal após duas reinjeções, além disso, foi registrado um maior valor médio no grupo reinjeção duas vezes e menor valor médio no grupo controle. Nos padrões de DRX não houve pico de difração referente à estrutura cristalina da resina acetal. Conclusão: os resultados do presente estudo concluíram que a reciclagem da resina de acetal aumentou a força retentiva do grampo e não afetou as estruturas cristalinas do material(AU)

ABSTRACT

Objective@#To investigate the effect of the socket-shield technique (SST) concurrent with immediate implant placement and provisionalization (IIPP) in the aesthetic restoration of anterior teeth.@*Methods@#A case of maxillary anterior tooth stumps with a thin labial bone wall was treated with SST for preservation of labial soft and hard tissue fullness, combined with an immediate implant placement and immediate provisional crown for restoring the shape of the tooth and gingival molding@*Results@#Immediate implant placement and provisionalization restored the morphology and function of the affected tooth in the shortest possible time. The patient's labial soft and hard tissue contours in the affected tooth area were well preserved in the 18-month follow-up after the application of the SST, which presented a better aesthetic result. The literature review indicates that the indications for SST are unrestorable maxillary anterior teeth, whose dental, periodontal and periapical tissues are healthy and intact. In the esthetic zone, root shielding is effective in maintaining the soft and hard tissue contour on the labial side of the implant. However, there is no consensus on the technical details of SST, such as the ideal coronal height and thickness of the shield, and the management of the gap between the shield and the implant. Thus, more clinical studies and histologic evidence are needed to provide a reference for clinical decision-making. In addition, digital technology can improve the accuracy of implant placement and shield preparation.@*Conclusion@#The correct application of SST combined with IIPP in the esthetic zone can ensure esthetic results. However, more high-quality evidence-based medical evidence is needed for its long-term efficacy, and indications should be strictly controlled during clinical application.

ABSTRACT

Background: Dental caries is the most prevalent disease in humans, especially during early childhood. The restoration of such an extensive carious lesion should be done properly to reestablish their anatomy and hence their masticatory, phonetic, esthetic, and space-maintainer functions in the dental arches. Composite resins are the most commonly used tooth-colored restorative material, but many difficulties occur particularly when used directly in posterior restorations. The objective of the study was to compare the clinical effectiveness of the Modified Open Sandwich Technique and Conventional Composite Resin Restoration for managing class II caries on primary molars. Material & Methods: This was a randomized control trial and was conducted in the Department of Pedodontics, Faculty of Dentistry, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh during the period from October, 2021 to September, 2022. In our study we included 68 patients with proximal dental caries of primary molars with reversible pulpitis. The patients were divided by using random sampling technique into two groups – Group A (The modified open sandwich technique, n=34) and Group B (The conventional composite resin restoration, n=34). Results: In total 68 patients from both the groups completed the study. In our study we found mean±SD of age was 5.37±1.24 years. 76.5% of cases of the modified open sandwich technique restorations were & 82.4% of the conventional composite resin restoration were < 6 years. Mandibular primary molars were 64.7% and 53% for the modified open sandwich technique and conventional composite resin restoration respectively. Conventional composite resin restorations showed 32.4% postoperative sensitivity and the modified open sandwich technique restorations showed 11.8%. After 12 months of follow-up, no secondary caries developed. Good marginal adaptation in 100% of cases were found in the modified open sandwich technique and 88.2% for conventional composite restorations. Conclusion: In our study we found that postoperative sensitivity developed more in conventional composite restorations in comparison to the modified open sandwich technique. Initially marginal adaptation showed better in the modified open sandwich method technique. So, conventional and modified open sandwich composite resin techniques were acceptable for primary molar teeth considering secondary caries and marginal adaptation

ABSTRACT

Therapeutic options for incisors affected by Molar Incisor Hypomineralization range from very conservative procedures to those considered to be more invasive. Clinicians must select the appropriate approach, considering the outcomes of the procedures. This case series presents clinical outcomes of five female patients with aesthetic complaints associated with the Molar Incisor Hipomineralization. Macroabrasion, microabrasion and dental vital bleaching were used as combination techniques in three cases. Resin infiltration was indicated for a specific case, considering the oncoming orthodontic treatment. Composite restoration was performed in one patient with posteruptive enamel breakdown. Clinicians must select the appropriate approach according to an individualized evaluation of each case, considering factors such as the patient's expectations, dental age, psychosocial period, the severity of the condition, presence of sensitivity and the presumed aesthetic result of the selected treatment. The advantages and disadvantages of the techniques are presented.


Las opciones terapéuticas para los incisivos afectados por hipomineralización incisivo molar van desde procedimientos muy conservadores hasta los considerados más invasivos. Los clínicos deben seleccionar el enfoque adecuado, teniendo en cuenta los resultados de los procedimientos. Esta serie de casos presenta los resultados clínicos de cinco pacientes femeninas con quejas estéticas asociadas a la Hipomineralización Incisivo Molar. En tres casos se utilizaron la macroabrasión, la microabrasión y el blanqueamiento dental vital como técnicas combinadas. La infiltración de resina fue indicada para un caso específico, considerando el tratamiento de ortodoncia próximo. Se realizó una restauración con composite en un paciente con rotura posteruptiva del esmalte. Los clínicos deben seleccionar el abordaje adecuado de acuerdo con una evaluación individualizada de cada caso, considerando factores como las expectativas del paciente, la edad dental, el período psicosocial, la gravedad de la afección, la presencia de sensibilidad y el resultado estético presumible del tratamiento seleccionado. Se presentan las ventajas e inconvenientes de las técnicas.

Introdução:O aprimoramento das resinas compostas nos últimosanos em associação com a difusão de informações nas redes sociais tornou as facetas diretas tratamentos populares na dentística restauradora. No entanto, são procedimentos que exigem ampla destreza manual e conhecimento técnico. O fluxo digital através doescaneamento, enceramento digital e prototipagem 3D para construção de guias tem se tornado uma excelente alternativa para aumentar a previsibilidade e aumentar a longevidade destes trabalhos. Objetivo:Descrever o protocolo de confecção de facetas diretas em resina composta, através de um relato de caso, utilizando como auxílio o planejamento digital para confecção de modelo 3D, guia de silicone e paredes palatinas. Descrição do Caso:Paciente do gênero masculino, 43 anos, queixava-se do formato dos seus dentes. Ao exame clínico percebeu-se desgaste dental nos incisivos centrais e linha do sorriso levemente invertida. Após duas sessões de clareamento de consultório com Peróxido de hidrogênio (35%) e mockup direto com resina composta, foi realizada a moldagem e escaneamento do modelo de gesso no laboratório. O enceramento digital foi aprovado, o modelo 3D foi impresso para confecção da guia de silicone. Com auxílio da guia foram executadas facetas diretas nos elementos 13, 12, 11, 21, 22 e 23. Conclusão:O fluxo digital pode ser uma alternativa viável para minimizar as falhas na confecção de facetas diretas em resina composta (AU).


Introduction:The improvement of composite resins in recent years, together with information disseminated on social media, has made direct veneers popular treatments in restorative dentistry. However, these procedures require significant manual dexterity and technical knowledge. Digital work flow using scanning, digital wax-up and 3D prototyping for the construction of guides has become an excellent alternative to increase predictability and the longevity of these procedures. Objective:Describe the manufacturing protocol for direct composite resin veneers, using a case report and digital to construct the 3D model, silicone guide and palatine walls. Case description:Male patient, 43 years old, complained of the shape of his teeth. Clinical examination revealed tooth wear on the central incisors and a slightly inverted smile line. After two whitening sessions with hydroigen peroxide (35%) and direct mockup with composite resin, the plaster model was molded and scanned in the laboratory. Digital wax-up was approved, and the 3D model was printed to manufacture the silicone guide. With the help of the guide, the direct veneers were applied to elements 13, 12, 11, 21, 22 and 23.Conclusions:Digital flow may be a feasible alternative to minimize manufacturing flaws in direct composite resin veneers (AU).


Introducción: La mejora de las resinas compuestas en los últimos años, y la difusión de información en las redes sociales, ha popularizado las facetas directas en los tratamientos en odontología restauradora. Sin embargo, son procedimientos que requieren demasiado destreza manual y conocimientos técnicos. El flujo digital usando escaneo, encerado digital y prototipado 3D para la construcción de guías se ha convertido en una excelente alternativa para aumentar la previsibilidad y la longevidad de estos procedimientos. Objetivo: Describir el protocolo para la realización de carillas directas en resina compuesta, a través de un reporte de caso, utilizando el planeo digital como ayuda para la realización de un modelo 3D, guía de silicona y paredes palatinas. Descripción del caso: Paciente masculino, 43 años, se quejó de la forma de sus dientes. El examen clínico reveló desgaste dental en los incisivos centrales y una línea de sonrisa levemente invertida. Después de dos sesiones de blanqueamiento en consultorio con peróxido de hidrógeno (35%) y maqueta directa con resina compuesta, el modelo de yeso fue moldeado y escaneado en el laboratorio. El encerado digital fue aprovado, el modelo 3D fue impreso para hacer la guía de silicona. Con la ayuda de la guía se realizaron carillas directas en los elementos 13, 12, 11, 21, 22 y 23. Conclusiones: El fluxo digital puede ser una alternativa viable para minimizar fallas en la fabricación de carillas directas en resina compuesta (AU).

Background: The objective of this systematic review and meta-analysis was to answer the questions ''Does lip repositioning surgery reduce the amount of gingiva exposed in the smile in individuals with excessive gingival display EGD (excessive gingival display)?'' and ''Is the reduction of the amount of gingiva exposed when smiling obtained with lip repositioning surgery stable over time?'' to evaluate the effectiveness of lip repositioning surgery for correcting EGD on smiling. Materials and Methods: A systematic structured search was carried out in five databases without data restriction. Studies reporting the degree of reduction in gingival ex-posure after lip repositioning surgery were included. Study selection, data extraction, and risk of bias assessment were performed in duplicate Results: One hundred sixty-four references were retrieved and eleven studies met the eligibility criteria. Meta-analysis demonstrated that lip repositioning surgery results in a significant reduction in the amount of exposed gingival tissue (mean difference = -3.03; confidence interval = -3.55; -2.52). In addition, the results remained stable in the evaluated follow-up periods (1, 3, 6 and 12 months). Included studies had a low risk of bias. Conclusions: Lip repositioning surgery is effective for the treatment of EGD on smiling and exhibits stable results in the evaluated periods. The study was registered (CRD42020184866) in the international prospective register of systematic reviews (PROSPERO)


Antecedentes: el objetivo de esta revisión sistemática y metanálisis fue responder a las preguntas "¿La cirugía de reposicionamiento de labios reduce la cantidad de encía expuesta en la sonrisa en personas con exposición gingival excesiva (EGE)?" y "¿La reducción de la cantidad de encía expuesto al sonreír obtenido con cirugía de reposicionamiento de labios estable en el tiempo?" para evaluar la efectividad de la cirugía de reposicionamiento de labios para corregir la EGE al sonreír. Materiales y Métodos: Se realizó una búsqueda sistemática estructurada en cinco bases de datos sin restricción de datos. Se incluyeron los estudios que informaron el grado de reducción de la exposición gingival después de la cirugía de reposición de labios. La selección de estudios, la extracción de datos y la evaluación del riesgo de sesgo se realizaron por duplicado. Resultados: Se recuperaron ciento sesenta y cuatro referencias y once estudios cumplieron con los criterios de elegibilidad. El metanálisis demostró que la cirugía de reposicionamiento de labios da como resultado una reducción significativa en la cantidad de tejido gingival expuesto (diferencia de medias = -3,03; intervalo de confianza = -3,55; -2,52). Además, los resultados se mantuvieron estables en los periodos de seguimiento evaluados (1, 3, 6 y 12 meses). Los estudios incluidos tenían un bajo riesgo de sesgo. Conclusión: La cirugía de reposicionamiento de labios es efectiva para el tratamiento de la EGE al sonreír y presenta resultados estables en los períodos evaluados. El estudio fue registrado (CRD42020184866) en el registro prospectivo internacional de revisiones sistemáticas (PROSPERO).

ABSTRACT

Introduction: The demand for cosmetic dentistry is a growing trend globally. Increased media coverage, availability of free online information, and the improved economic status of the general population have led to an increase in the patient’s aesthetic expectations. Since no study has measured the relationship between the economic status of people and the choice of cosmetic dentistry in Iran and due to the growing demand for it, this study was designed. Method: In this descriptive epidemiological study, we visited three completely different areas in terms of Socioeconomic status (SES) in Tehran. A checklist was used to record the gender, job, age, education level, maternal status, and source of payment for cosmetic dentistry treatment. Result: The majority of the volunteers for dental cosmetic restoration were 23?26 years. Of 498 cosmetic restoration volunteers, 50 were men and 448 were women. As for the education level, most of the participants had a high school diploma. The cost of cosmetic restoration of 351 patients (70%) was paid by the parents or the spouse, and 147 patients paid the cost from their income. Our study showed that 7% of visits to dental clinics were only for cosmetic treatment demands in Tehran in 2021. Conclusion: Type of job, education level, and marital status had no significant relationship with the choice of cosmetic treatment while age had a significant relationship with the choice of cosmetic dental restoration treatment. In addition, the choice of cosmetic dental treatment was completely related to gender, and women were the main users of it.

ABSTRACT

To determine the clinical performance of zirconia abutment (ZA) by comparing with a titanium abutment (TA) and sub?mucosal?modified zirconia abutment. A systematic search was conducted to retrieve eligible randomised controlled trials (RCTs) from Medline, Cochrane Library, SCOPUS, Embase, Web of Science, and Google Scholar. A search was further divided in two parts. Part I comprises eligible RCTs between zirconia abutment and titanium abutment, and part II included RCTs of zirconia abutment with sub?mucosal modified, pink?veneered glass ceramic versus non?veneered zirconia abutment. Esthetic, biological, and abutment survival was a primary outcome, and technical complications were included as an additional outcome. Fifteen eligible RCTs (Part I: N = 9 and Part II: N = 6) were evaluated, and a total of 362 abutments in 364 subjects were analysed for outcome variables. A sub?group meta?analysis reported no significant difference for Esthetic outcome. However, the overall mean (p =0.03) was higher for zirconia group in those of thin gingival phenotype. Spectrophotometric evaluation of peri?implant mucosal Esthetic does not show any significant difference. Similarly, pink?veneered versus non?veneered group reported no significant difference for thin (<2 mm) and thick (>2 mm) mucosal attachment. Biological outcome does not show any significant difference for comparable groups in both parts. There is marginally lower abutment survival for internally connected zirconia abutment (ZA: 95.4% TA: 100%). Objective@#To explore the ideal sagittal position of the lower third of the face in high-angle patients with different forehead forms and to provide a reference for clinical treatment. @*Methods@#Informed consent and portrait authorization were obtained from all patients, and the study passed the ethical review of the unit. We categorized forehead forms into four types: straight, rounded, type I angular (angled at the middle third of the forehead) and type II angular (angled at the upper third of the forehead). Profiles of high-angle patients with different forehead forms were collected. The initial position was when the facial axis point (FA point) was positioned at the goal anterior-limit line (GALL). After being silhouetted, the lower third of the face was moved forward and backward by 1 mm, 2 mm, 3 mm, and 4 mm each, plus the initial silhouetted picture, to obtain 9 images for each patient. A survey was created with these lateral profile silhouettes, and the silhouette images were ranked by 30 orthodontists and 30 laypersons. @*Results@# There were significant differences in profile scores at different movement distances of the lower third of the face among high-angle patients with different forehead shapes (P<0.05). Overall, high-angle patients with straight or type II angular foreheads had higher scores when the lower third of the face did not move. For high-angle patients with a rounded forehead, orthodontists and laypersons gave the highest scores when the lower third of the face was moved backward by 2 mm and 4 mm, respectively. For high-angle patients with a type I angular forehead, orthodontists thought the scores of backward movement of 4 mm were the highest, and laypersons thought the scores of backward movement of 3 mm were the highest. No significant difference was found in scores between orthodontists and laypersons (P>0.05). @*Conclusion @#The forehead forms and the sagittal position of the lower third of the face will affect the face’s profile aesthetics. Patients with straight and type Ⅱ angular foreheads has the best profile when the FA point is located on the GALL line. For patients with rounded and type Ⅰ angular foreheads, a posterior location of the lower third of the face is more desirable than the initial position.

@#After tooth extraction, labial contour collapses due to inevitable physiologic bone remodeling. To achieve optimal outcomes for pink esthetic treatment at anterior implant sites, bone or soft tissue augmentation has been advocated to maintain or reconstruct the labial tissue contour. When choosing soft tissue augmentation for esthetic restoration, it is necessary to strictly grasp the indications for surgery. Soft tissue augmentation to maintain or reconstruct the labial tissue contour could be considered in patients with healthy soft tissue and no bone defects or only mild horizontal bone defects. In immediate, early and late implant placement, the timing of soft tissue augmentation may vary. In immediate implantation, the labial bone plate is intact, so it is highly recommended to simultaneously manage soft tissue during implant placement. However, patients may have large bone defects with early or late implant placement. The risk of augmenting bone and soft tissue simultaneously is likely too high, and bone augmentation surgery is often performed at the first stage while soft tissue augmentation surgery is performed at the second stage. Therefore, soft tissue surgery is often carried out simultaneously with abutment connection. Currently, soft tissue augmentation is achieved mostly with adjacent autologous soft tissue grafts, such as free gingival grafts, subepithelial connective tissue grafts or pedicle palatal flaps, which are often accompanied by a second surgical area. The replacement of autogenous soft tissue grafting with new biological materials will become an inevitable trend. In this article, we analyze and summarize the indications, timing and different methods of soft tissue augmentation to maintain and reconstruct the labial contour.

Este estudio busca describir elementos de masculinidad y feminidad relacionados en el ingreso y el accionar en una actividad delictiva de alto impacto: el tráfico de drogas. Respetando una metodología cualitativa, se utilizó una entrevista semiestructurada a un grupo de personas que se dedicaron al tráfico de drogas en el estado de Sonora, México, dos mujeres y siete hombres, para explorar elementos de feminidad y masculinidad relacionados con el ingreso al tráfico de drogas en cuatro dimensiones: poder, economía, estética y cuerpo. Se muestra que los analizados buscan en el tráfico cumplir con modelos impuestos socialmente en función de su género; existen también diferenciaciones que sugieren una revalorización en sus maneras de ejercer el poder, sus metas económicas y el uso del cuerpo para obtener cuotas de poder.


This study seeks to describe elements of masculinity and femininity related to entering and acting in a high-impact criminal activity: drug trafficking. Following a qualitative methodology, a semi-structured interview was used with a group of people involved in drug trafficking in the state of Sonora, Mexico, two women and seven men, to explore elements of femininity and masculinity related to entering drug tra-fficking in four dimensions: power, economy, aesthetics and body. It is shown that those analyzed seek in trafficking to comply with socially imposed models based on their gender; there are also differentiations that suggest a revaluation in their ways of exercising power, their economic goals and the use of the body to obtain power quotas.


Este estudo procura descrever elementos de masculinidade e feminilidade relacionados à entrada e operação em uma atividade criminosa de alto impacto: o tráfico de drogas. Seguindo uma metodologia qualitativa, foi utilizada uma entrevista semi-estruturada com um grupo de pessoas envolvidas no tráfico de drogas no estado de Sonora, México, duas mulheres e sete homens, para explorar elementos de feminilidade e masculinidade relacionados ao ingresso no tráfico de drogas em quatro dimensões: poder, economia, estética e corpo. Mostra-se que os analisados procuram no tráfico cumprir modelos socialmente impostos com base em seu gênero; há também diferenciações que sugerem uma revalorização em suas formas de exercer o poder, seus objetivos econômicos e o uso do órgão para obter cotas de poder.

ABSTRACT Introduction: Although the superiority of clear aligners over multi-bracket appliances in keeping gingiva healthy has been suggested, the possible benefits of one aligner design over another have not yet been investigated, especially with regard to the vestibular edge. Objective: The aim of this study was to measure several periodontal indexes in adolescents undergoing orthodontic treatment with aligners, comparing two different types of rim. Methods: The study involved 43 patients aged between 14 and 18 years. The periodontal health was assessed using plaque index (PI), gingival index (GI), and gingival bleeding index (GBI), at the start of the treatment with aligners (T0), with a vestibular rim (VR) reaching up to 3 mm beyond the gingival margin. Three months later (T1), aligners were set to obtain a juxtagingival rim (JR) on the second quadrant and VR on the first quadrant. The periodontal indexes were measured again, both at T1 and then three months later (T2). Results: Intra-quadrant comparisons revealed a statistically significant worsening of the periodontal indexes only for the second quadrant (p<0.05), at T1 (GI), and especially at T2 (PI, GI, GBI), while no statistically significant changes were found for the first quadrant. Conclusions: More severe mechanical irritation, especially during insertion and removal of the aligner, can explain the worsening inflammatory indexes with the JR. In addition, the pressure exerted by the JR on the gingival sulcus seemed to facilitate plaque deposition, whereas the VR had a protective effect, reducing the risk of mechanical trauma.


RESUMO Introdução: Embora se alegue uma superioridade dos alinhadores transparentes sobre os aparelhos multibraquetes em manter a gengiva saudável, ainda não foram investigados os possíveis benefícios de um modelo de alinhador sobre outro, especialmente no que diz respeito à borda vestibular. Objetivo: O objetivo deste estudo foi medir vários índices periodontais em adolescentes submetidos a tratamento ortodôntico com alinhadores, comparando dois tipos diferentes de borda. Métodos: O estudo envolveu 43 pacientes com idade entre 14 e 18 anos. A saúde periodontal foi avaliada por meio do índice de placa (IP), índice gengival (IG) e índice de sangramento gengival (ISG), ao início do tratamento com alinhadores (T0), com borda vestibular (BV) estendendo-se até 3 mm além da margem gengival. Três meses depois (T1), os alinhadores foram ajustados para se obter uma borda justagengival (BJ) no segundo quadrante e BV no primeiro quadrante. Os índices periodontais foram medidos novamente tanto em T1 quanto três meses depois (T2). Resultados: As comparações intraquadrantes revelaram uma piora estatisticamente significativa nos índices periodontais apenas para o segundo quadrante (p<0,05), em T1 (IG) e especialmente em T2 (IP, IG, ISG), enquanto nenhuma mudança estatisticamente significativa foi encontrada para o primeiro quadrante. Conclusões: Irritações mecânicas mais intensas, principalmente durante a inserção e remoção do alinhador, podem explicar a piora dos índices inflamatórios com a BJ. Além disso, a pressão exercida pela BJ no sulco gengival pareceu facilitar a deposição de placa, enquanto a BV teve um efeito protetor, reduzindo o risco de trauma mecânico.

ABSTRACT The gingival contour, the color of the teeth and the presence of interincisal diastema are seen as unsightly factors, being highly harmful from a social point of view. The present study aimed to report a comprehensive and conservative esthetic treatment executed with a multidisciplinary approach in a 20-year-old female patient that presented darkened teeth with poor spacing and excessive gingival display while smiling, as well as present a brief literature review on multidisciplinary approach in esthetic dentistry. After clinical examination and diagnosis, a treatment plan was elaborated to meet the patient's expectations with a conservative approach. After patient consent, therapy began with supervised home-based tooth whitening with 22% carbamide peroxide applied one hour/daily for 14 days, followed with gingival recontouring surgery and, after a healing period of 30 days, diastemas closure with direct composite resin technique, occlusal adjustment, finishing, and polishing. The multidisciplinary approach was fundamental for the accomplishment of the proposed treatment, achieving satisfactory esthetic and functional results.


RESUMO O contorno gengival, a cor dos dentes e a presença de diastemas interincisivos são vistos como fatores antiestéticos, sendo altamente prejudicial do ponto de vista social. O presente estudo teve como objetivo relatar um tratamento estético conservador executado com abordagem multidisciplinar em uma paciente do sexo feminino de 20 anos que apresentava dentes escurecidos com pouco espaçamento e exposição gengival excessiva ao sorrir, bem como apresentar uma breve revisão de literatura sobre à abordagem multidisciplinar em odontologia estética. Após exame clínico e diagnóstico, foi elaborado um plano de tratamento para atender as expectativas do paciente com abordagem conservadora. Após consentimento do paciente, foi iniciada o clareamento dental supervisionado com peróxido de carbamida 22% aplicado uma hora/dia por 14 dias, seguido de cirurgia de recontorno gengival e, após um período de cicatrização de 30 dias, fechamento de diastemas com técnica de resina composta direta. Neste caso, a abordagem multidisciplinar foi fundamental para a realização do tratamento proposto, alcançando resultados estéticos e funcionais satisfatórios.

La técnica CAD/CAM es un sistema que ha facilitado los procesos de toma de impresiones totales e incorpora registros intraorales y requisitos apropiados para lograr bases protésicas adecuadas, lo cual permite óptimos resultados en clínica dental, como son el ahorro de tiempo y la comodidad para el especialista y el paciente, razones que llevan al profesional estomatológico a dar el paso hacia una odontología digital. El objetivo del estudio fue analizar el sistema CAD/CAM en la confección de prótesis totales dentales. La búsqueda se realizó en las bases de datos Google Scholar, Pubmed y Scopus, intentando responder a la pregunta orientadora: ¿Cuáles son los aspectos más significativos para el sistema CAD/CAM en la confección de prótesis totales dentales? Como resultados, se pudo interpretar que, consiguiendo registros tridimensionales producidos en un escáner, se proporciona una mejor adaptación de estructuras respecto a los métodos convencionales, por lo que induce estética y precisión, que son factores importantes en la odontología. Se concluyó que el sistema CAD/CAM disminuye el margen de error humano y conduce a mayores éxitos odontológicos, al brindar un mejor soporte para la toma decisiones y conseguir mejores resultados estéticos y funcionales en las prótesis totales de los pacientes(AU)


The CAD/CAM technique is a system that has facilitated the process of taking total impressions and incorporates intraoral records and appropriate requirements to achieve adequate prosthetic bases, which allows optimal results in the dental clinic, such as time savings and comfort for the specialist and the patient, reasons that lead the dental professional to take the step towards digital dentistry. The objective of the study was to analyze the CAD/CAM system in the fabrication of total dental prostheses. The search was carried out in Google Scholar, Pubmed and Scopus databases, trying to answer the guiding question: What are the most significant aspects for the CAD/CAM system in the manufacture of total dental prostheses? As results, it was possible to interpret that, by obtaining three-dimensional records produced in a scanner, it provides a better adaptation of structures compared to conventional methods, thus inducing esthetics and precision, which are important factors in dentistry. It was concluded that the CAD/CAM system reduces the margin of human error and leads to greater dental success by providing better support for decision making and achieving better esthetic and functional results in the total prostheses of patients(AU)

ABSTRACT

@#Gingival recessions (GRs) result in root hypersensitivity, root surface caries, and esthetic problems. Various root coverage surgeries are being developed for periodontal plastic therapy. The tunnel technique (TUN) is one of the most widely applied surgeries due to its features of being minimally invasive, practical, excellent outcomes and long-term stability; however, there are still some limitations of this technique. The history and evolution from the envelope flap to TUN, including its efficiency when compared with coronally advanced flaps with a connective tissue graft (CTG), are reviewed in this paper. The limitations of TUN are discussed in consideration of our clinical experience; for example, there is high technique sensitivity when TUN is applied in GR>5 mm because of the great difficulty in covering the grafts. The advantages of surgical access, including vertical incisions in the vestibule, “W” type and pinhole access, are discussed for different situations. Mattress sutures and sling sutures in a single tooth or multiple teeth are applied in TUN. The different types of grafts, such as CTG, platelet-rich plasma, articular dermal matrix and xenogeneic collagen matrix, are described. Mechanical, chemical and biological conditioning of the root surface are recommended during surgery. Protecting the surgical area and taking antibiotics postoperatively are also very important. Finally, the modifications when TUN is applied with other kinds of techniques are discussed, including lateral closed TUN, laterally positioned flaps, double papilla flaps and frenuloplasty. Minimally invasive, esthetic, long-term stability and simplified techniques are the development trends of TUN in the future.

Introducción. La cirugía estética tiene un importante reconocimiento en la sociedad contemporánea. Colombia se ubica como uno de los países en el que anualmente se practica un alto número de procedimientos estéticos invasivos; sin embargo, también se ha establecido que esta especialidad presenta frecuentes complicaciones cuyos riesgos necesitan ser registrados y sistematizados. Objetivo: Proponer un modelo adicional de sistematización de riesgos en cirugía estética que fortalezca los comúnmente empleados en Colombia. Metodología: El método utilizado fue el de estrategias de prevención de riesgos aplicadas al ámbito médico, empleando para este trabajo las dos primeras etapas, es decir, la sistematización de fuentes de información a partir de datos obtenidos de la gaceta jurisprudencial publicada por el Tribunal Nacional de Ética Médica y la identificación de la taxonomía de los eventos y errores médicos en cirugía estética. Resultados: En total se analizaron 57 providencias de las gacetas del Tribunal Nacional de Ética Médica sobre cirugía plástica y estética (privilegiando los procedimientos estéticos invasivos), pertenecientes a 2012, 2015 y 2017; de estas 34 cumplieron los criterios de selección; posteriormente, se realizó la sistematización de los riesgos hallados. Conclusiones: El modelo de tecnovigilancia empleado en Colombia resulta insuficiente a la hora de registrar los eventos y errores que se producen en procedimientos estéticos invasivos, por lo que es necesario emplear métodos adicionales que permitan garantizar una mayor seguridad en el paciente, prevenir daños lesivos y disminuir los procesos judiciales en contra del personal sanitario.


Introduction: Plastic surgery has an important place in today's contemporary society. Colombia ranks as one of the countries with the highest annual number of performed invasive esthetic procedures. However, it has been established that this type of special procedures showcases frequent complications and the need to register and systematize risks. Objective: To propose an additional model of risk systematization in cosmetic surgery that strengthens those commonly used in Colombia. Methodology: The method used was that of risk prevention strategies applied to the medical field, using for this work the first two stages, that is, the systematization of sources of information from data obtained from the jurisprudential gazette published by the National Court of Medical Ethics, and the identification of the taxonomy of events and medical errors in esthetic surgery. Results: In total, 57 orders of the National Court of Medical Ethics on plastic and esthetic surgery (favoring invasive esthetic procedures), from 2012, 2015, and 2017, were analyzed, of which 34 met the selection criteria; later, the found risks were systematized. Conclusions: The techno-vigilance model used in Colombia is insufficient to record events and errors that occur in invasive esthetic procedures, so it is necessary to use additional methods to ensure greater patient safety, prevent harm, and reduce judicial proceedings against healthcare personnel.

Resumen La Fluorosis Dental (FD) es un defecto del desarrollo del esmalte tipo hipomineralización atribuido a la exposición crónica y excesiva de fluoruro durante el período de amelogénesis. Clínicamente, en los casos más leves se pueden observar finas líneas blancas y opacas, mientras que en casos moderados y severos las líneas se pueden fundir y formar bandas más gruesas comprometiendo toda la superficie del diente pudiendo estar asociadas con pérdidas de estructura y cambios de color. El tratamiento de la FD puede incluir desde microabrasión, infiltración, aclaramiento y restauraciones con resina compuesta hasta carillas y coronas. El objetivo de este caso clínico fue asociar la técnica de microabrasión e infiltración como alterantiva de tratamiento estético para el manejo de la FD. Paciente de sexo femenino y 9 años que presentaba opacidades blancas en forma de bandas gruesas y pérdidas de estructura dental en los incisivos superiores permanentes (FD severa). Para el tratamiento se optó por realizar microabrasión con ácido clorhídrico, seguido de infiltración con resina de baja viscosidad, logrando un resultado estético favorable y estable a tres meses. Se concluye que la combinación de estas dos técnicas micro-invasivas es una estrategia viable y efectiva para enmascarar efectivamente opacidades producidas por la FD.


Resumo A Fluorose Dentária (FD) é um defeito de desenvolvimento do esmalte do tipo hipomineralização atribuído à exposição crônica e excessiva de fluoreto durante a amelogênese. Clinicamente, os casos leves apresentam finas linhas brancas que acompanham a formação dentária, enquanto casos moderados ou severos apresentam faixas brancas mais amplas acometendo toda a superfície e podem estar associados à perdas estruturais e pigmentações. O tratamento da FD inclui desde microabrasão, infiltração e aclaramento até restaurações com resina composta, facetas e coroas. O objetivo deste caso clínico foi associar as técnicas de microabrasão e infiltração como alternativa de tratamento estético para a abordagem da FD. Paciente do sexo feminino de 9 anos que apresentava nos incisivos superiores permanentes áreas brancas opacas e perda de estrutura (FD severa). Para o tratamento, optou-se por realizar microabrasão com ácido clorídrico, seguido de infiltração com uma resina de baixa viscosidade, atingindo um resultado estético e favorável. Conclui-se que a combinação da microabrasão e da infiltração é uma estratégia viável e efetiva para mascar efetivamente opacidades produzidas pela FD.


Abstract Dental Fluorosis (DF) is a developmental defect of the enamel type hypomineralization attributed to chronic and excessive fluoride exposure during the period of amelogenesis. Clinically, in the mildest cases fine white and opaque lines can be observed, while in moderate and severe cases the lines can melt and form thicker bands, compromising the entire tooth surface, which may be associated with loss of structure and color changes. Treatment of DF can range from microabrasion, infiltration, dental whitening, and composite resin restorations to veneers and crowns. The objective of this clinical case was to associate the microabrasion and infiltration technique as an alternative to aesthetic treatment for the management of dental fluorosis. A 9-year-old female patient presented white opacities in the form of thick bands and loss of tooth structure in the permanent upper incisors (severe dental fluorosis). For the treatment, it was decided to perform microabrasion with hydrochloric acid, followed by infiltration with low viscosity resin, achieving a favorable and stable aesthetic result after three months. It is concluded that the combination of these two micro-invasive techniques is a viable and effective strategy to effectively mask opacities produced by DF.

Increased patient demands for highly esthetic implant superstructure in the anterior esthetic zone has increased in the last decades. Moreover, the absence of periodontal ligament in implant supported prosthesis causes forces to be transferred without cushioning effect to the alveolar bone, resulting in increased marginal bone loss (MBL) and influence the heath of peri-implant tissue. Evaluate the available evidence on the effect implant superstructure and it consequences on patient satisfaction, MBL, bleeding on probing (BOP) and probing depth (PD). A protocol of electronic and hand research was performed for English based researches comparing implants inserted in the esthetic zone with all ceramic superstructure: "Will the use of different types of all ceramic superstructure show different esthetic patient satisfaction, marginal bone loss, bleeding on probing and probing depth? Thirteen publications from one thousand one hundred and sixteen research studies were included. This systematic review showed that all ceramic implant superstructure was a versatile treatment option with higher esthetic patient satisfaction and better color of peri-implant mucous especially in patients having thin biotype. On the other hand there wasn't significant difference in MBL, PD and BOP compared to other conventional implant superstructure. More randomized controlled clinical trials with bigger samples are needed to confirm our findings. All ceramic implant superstructure is versatile and highly esthetic treatment option for implant placed in the anterior esthetic zone.(AU)


A demanda do paciente por superestruturas de implante altamente estéticas na zona anterior aumentou nas últimas décadas. Além disso, a ausência de ligamento periodontal em próteses implantossuportadas faz com que as forças sejam transferidas para o osso alveolar sem amortecimento, resultando em aumento da perda óssea marginal (MBL) e influenciando na saúde do tecido peri-implantar. Avaliar as evidências disponíveis sobre o efeito da superestrutura do implante e suas consequências na satisfação do paciente, perda óssea marginal, sangramento à sondagem (SS) e profundidade de sondagem (PS). Um protocolo de pesquisa eletrônica e manual foi realizado para a análise de artigos em inglês comparando implantes com toda a superestrutura em cerâmica inseridos na zona estética: "O uso de diferentes tipos de superestrutura em cerâmica mostrará diferentes níveis de satisfação estética do paciente, perda óssea marginal, sangramento em profundidade de sondagem e sondagem?". Foram selecionadas 1116 publicações e apenas treze estudos foram incluídos na análise final. Esta revisão sistemática mostrou que toda superestrutura do implante em cerâmica foi uma opção de tratamento versátil, com maior satisfação estética do paciente e melhor coloração da mucosa peri-implantar, especialmente em pacientes com biótipo fino. Por outro lado, não houve diferença significativa em MBL, PS e SS em comparação com outras superestruturas de implantes convencionais. Porém, mais ensaios clínicos controlados randomizados com amostras maiores são necessários para confirmar nossos achados. Implicações clínicas: Toda superestrutura do implante em cerâmica é uma opção de tratamento versátil e altamente estética para implantes colocados na zona anterior (AU)

Objective@#To study the postoperative soft and hard tissue changes and aesthetic effect of immediate implantation and provisionalization (IIPP) combined with guided bone regeneration (GBR) for a single anterior maxillary tooth with a thin facial bone phenotype.@*Methods @# A total of 34 patients with thin facial bone (<1 mm) were categorized into two groups: a flapped GBR group and a flapless group. Tooth extraction and IIPP were conducted at the sites in both groups. Implant survival rates, dimensional changes in soft and hard tissues during the six- and twelve-month follow-ups, the pink esthetic score (PES) and patient satisfaction scores at the twelve-month follow-up were measured.@*Results @#The implant survival rates were 100% in both groups, and no complications occurred during the 12 months after surgery. The facial bone thickness remained over 2 mm on all measured sides, and the height of the facial bone crest remained at 1.39 mm at the 12-month follow-up in the flapped GBR group, while the facial bone thickness remained less than 2 mm on all measured sides, and the height of the facial bone crest remained at 1.03 mm at the 12-month follow-up in the flapless group. The absorption of facial bone at all measured sides in the flapped GBR group was greater than that in the flapless group (P<0.05). There was no significant difference between the two groups in the dimensional changes of labial soft tissues during the six- and twelve-month follow-ups (P>0.05). The mean PES scores were 10.29 ± 2.34 for the flap GBR group and 10.12±1.78 for the flapless group (P = 0.807). The mean patient satisfaction scores were 8.65 ± 1.27 in the flapped GBR group and 8.76 ± 1.25 in the flapless group, and the patients in both of the groups were satisfied with the esthetic outcomes (P = 0.787). @*Conclusion @#IIPP combined with GBR might be a prospective treatment strategy for a single anterior maxillary tooth with a thin facial bone phenotype, but the esthetic risks should never be ignored.

ABSTRACT

Genital dissatisfaction is well known in female and adults. Less is known about male adolescents and their genital satisfaction. The aim of this study was to investigate and report the role of surgery in male adolescents to improve the evaluation of their genitalia. We considered all patients treated for external genital pathology in the period of adolescence. Inclusion and exclusion criteria were created. Patients underwent an evaluation test before and after surgery. During the study period, 137 patients were treated, and at the end of the study, 98 cases were considered for analysis. The most frequent pathologies were webbed penis and penile curvature. A postoperative score improvement was noted and patients with concealed penis and webbed penis showed a better postoperative outcome. Overweight was considered an important factor associated with a worse preoperative score. Evaluation of the external genitalia is important in adolescents, and it is an understudied problem. Overweight may be associated with a worse evaluation of one's genital and should be clinically considered to avoid related social problems in adulthood. Therefore, cosmetic genital surgery should be considered even in male adolescents.
